This information is the user id and password that you would like to use with your PTL online account. The rules regarding what can be used in a user id and password are:

Must contain only numbers (0-9) and letters (A-Z)
Must start with a letter (A-Z)
Minumum length of 6 characters
Maximum length of 10 characters
